As a Federally Qualified Health Center, Advantage Health provides a Sliding Fee Discount Schedule (SFDS) to its patients. This is determined by a patient’s ability to pay and ensures that receiving quality health care and services are never a burden to you as a patient. The SFDS will be clearly explained to you when registering and at any time you need further information or clarification. Please provide a proof of income upon your first visit to establish the appropriate sliding fee for yourself and your family. This is reevaluated once a year.
